Flex Spending 1- Enrollment Form

Form must be submitted to Human Resources for processing. A Flexible Spending Account (FSA), also called a flex plan or reimbursement account, is an employer‐sponsored benefit that allows you to set aside funds on a pre‐tax basis to pay for eligible out-of-pocket medical expenses. If you expect to incur medical expenses that will not be paid by your regular health insurance plan, you should be taking advantage of the flexible spending account. There are also similar accounts for dependent and child‐care expenses. There are two types of spending accounts: Medical Health Care Flexible Spending Account (HCFSA)- $2,550 Limit for Calendar Year 2016 Pre-tax funds to pay out‐of‐pocket medical expenses (co-pays, deductibles, vision care expenses, non-cosmetic dental care, medically necessary prescriptions)‐– Expenses can be for participant, spouse or any dependent claimed on the participant’s income tax return (regardless of whose insurance the dependent is on). Expenses for domestic partners are not eligible. Dependent Care Flexible Spending Account (DCFSA) $5,000 Limit for Calendar Year 2016 Pre-tax funds to pay for- 1) Childcare expenses (Children under the age of 13 and entitled to a dependent deduction under IRS) 2) Expenses for care of a dependent who is physically or mental incapable of caring for himself or herself 3) Elder care expenses Eligible dependent care expenses include: Before/after school care, day care, summer camp, late pick-up fees, registration and application fees (these are not reimbursable until care has been provided) Program Rules for Medical Flex Spending Account & Dependent Care Flex Spending Account • Employees wishing to participate must enroll in the program; • Domestic partner expenses will not be covered under this program; • Start with a small if you are unsure of how much you will actually spend; • This is a lose it or use it program, so plan accordingly (see attached planning worksheet); • Expenses incurred through March 15, 2017 are eligible for reimbursement from the 2016 plan year. *SNH *FNH
